Initial AppointmentWhen you come for your first appointment, you will have an examination and consultation with the physician.

It is very important you bring the following items with you:

  • Most recent mammogram and breast ultrasound films.
  • Dictated mammogram and ultrasound reports.
  • Pathology reports for any previous breast surgeries.
  • List of current medications and dosages.
  • Your insurance card.
  • Completed patient registration and medical forms.
