INSURANCE & PAYMENT

Payment for professional services, including deductibles or co-payments, is expected at the time of service. Payments may be made by cash, check, Visa or Mastercard.

We file insurance with several companies as an additional service to you. Please remember your medical insurance policy is a contract between you and your insurance company, therefore we cannot guarantee their payment of your claim.

If you are not sure about the requirements of your insurance company regarding referral, please check your insurance card for instructions. If a doctor’s name appears on the front of your card it usually means you must have an insurance referral to us.

Please speak with your benefits department at your employment or all your insurance company for instructions regarding insurance authorization referrals.
